What documents are required for, "CHANGE OF NAME", in passport? OR What is procedure to change name in passport?


 CHANGE OF NAME

I.    Following marriage, remarriage or divorce:

(a)        A woman applying for change of name/surname in existing passport due to marriage must furnish:

(i)      Photocopy of the Husband’s passport, if any, and
(ii)    An attested copy of marriage certificate issued by Registrar of Marriage OR an affidavit from the husband and wife along with a joint photograph, (Specimen at Annexure D).

(b)        Divorcees applying for change of name OR for deletion of spouse’s name in existing passport must furnish

 (i)   Certified copy of Divorce decree.
(ii)    Deed poll/sworn affidavit (ANNEXURE ‘E’)

(c)  Re-married applicants applying for change of name/spouse’s name must furnish:

 (i)  Divorce deed/death certificate as the case may be in respect of first spouse, and
(ii)  Document as at (a) above relating to second marriage.

      II. In other circumstances for change of name, the applicant (both male and female) should furnish:                
    (i)  Deed poll/sworn affidavit (ANNEXURE ‘E’):
    (ii) Paper cuttings of two leading daily newspapers in original (one daily newspaper should be of the area of applicant’s permanent and present address or nearby area).

In my passport ( which was expired in 2002), my mother's name is wrong and now i want to renew my passport, please tell me how i can rectify my mothers' name in my new/renewal of passport. if I have to give affidavit then what sort of wording is required for that .

You must be having supporting documents in which your mother's name is correct. e.g. matriculation certificate, Degree, id proof etc.

If the mistake was done by passport office itself, then you need not to provide any sort of affidavit. Just fill a new applcation form, mention, "Re-issue" in the above box. Fill up simple as per the format. There you have to mention your previous passport no. in the requisite box and you need to provide your old passport along with the application form. If asked, then simply let them know that you have the supporting documents and the mistake was done at their end. No issues thereafter.

Don't worry. It can be corrected.

You don't need to re-apply.

Just visit your regional passport sewa kendra.

You need to submit FORM no. 2 with supporting documents. Generally, such corrections are done within same day.

The authority will correct it manually. They will fix a seal and signature so that it should be acceptable everywhere.

Answer: If you have applied online, then there would be no issue. Simply create a new account and use that. No worry with the existing one. You can deactivate the existing account.

But, if you have applied physically by visiting Regional passport office, then you can't cancel it unless you should be suggested by Regional passport officer to apply with a new application form.

Can you elaborate more why you need to do so? May be we could solve this issue more precisely once if we know what exactly the problem is.

hello mam
i hve dne my appointment round bt due to some problem in the documents dey asked me 2 resubmit it so i wnt 2 ask i hve 2 agn go throught all d procedure and is the passport office open on saturdays

@ Pratibha.........kindly tell whether your all documents were submitted and did you receive any receipt from passport office? If yes, then you need not to go for whole procedure only you need to submit the valid documents they require but if, your application along with documents are not accepted by them and you didn't get any receipt that means till now your application is not received by Passport office then you need to go through all the process. Sorry to say but passport offices remain closed during weekends Nothing to worry, take appointment as per your convenience and then submit your remaining documents.

@ Pratibha, I am a male candidate applying for the deletion of the spouse name due to divorce. i have a Indian court decree. I am not finding the relevance of Annexure E document in my case ? how do we take that format - as Indian male normally do not have any name change after divorce, which is not so with the females. So, is it mandatory to have Annexure E for males too along with Court Decree letter ?? How to go about Annexure E in general/ in specific case like this!! Pls advise

Hi
I want to remove my ex wife's name from my passport as we have divorced. In the documents submission list there it is shown that i have to give annexure E. But annexure E is all about change in name and nothing to do removing spouses name. Is there any other annexure E which can be used in my case. Thanks

Respected Madam,

I am applying for reissue of my passport. My previous passport the marital status was single; now as I'm applying for reissue of passport (after 10 years) I'm divorcee. I searched the website and figured that i would have to submit a divorcee decree copy. Also it mentions that i have to submit Annexure E. Do i really need to submit the Annexure E as there would be no change in my name and as my previous passport didn't have spouse name, the new passport would also not have it. ?

If i need to submit Annexure E, where can i get it done - a format or specimen copy would help.

My case is very similar where I dont have surname change, but just spouse name removal.
Can you please guide me on whether you had submitted Annexure E without surname change and just for spouse name removal?

@ Uttam Kumar ......Hi, Yes Annexure E need to be furnished if CHANGE IN NAME is desired. Change in name is never mandatory in Passport application unless someone wants to make it.
