Before applying for a passport, applicants must carefully review the following important points to ensure a smooth application process:
✅ Address Proof Requirements
-
Applicants must provide at least two address proofs showing residency at the current address for more than one year.
-
For example, if applying in May 2010, the documents should reflect residence from April 2009 or earlier.
✅ Consistency in Personal Details
-
Ensure that the name of the applicant, father’s name, mother’s name, and address are consistent across all documents and match the spelling and surname used in the application form.
-
Any discrepancies may lead to objections or delays.
✅ Photograph Specifications
-
Submit recent passport-size photographs with a light or white background.
-
Outdated or incorrect photo formats may result in rejection.
✅ Original Documents Must Be Carried
-
Always carry original documents when submitting the passport application.
-
These will be verified on the spot and returned immediately.
-
Failure to present originals is a common reason for rejection and repeat visits.
✅ Ensure Accuracy in Application Details
-
All information provided in the application form must be accurate and truthful.
-
Any false or incorrect details may lead to penalties under passport regulations.
✅ Proof of Date of Birth Requirements
-
For applicants born before 1989:
-
Submit a self-attested copy of either the matriculation certificate or school leaving certificate.
-
Illiterate applicants may submit a Notary-attested affidavit.
-
-
For applicants born on or after 1989:
-
Only a birth certificate issued by the municipal corporation will be accepted.

Hi
I cam across your blog. I am a student and want to apply for fresh passport online. while filling up my parents name i have put in the order : Name Surname. But in my personal particulars form surname has to followed by the name for my parents. I am confused whether i should put the surname first or the name while filling up the online passport form.
waiting for your reply at the earliest.
Thanks & Regards,
Soumya Chatterjee
@Rahul kumar:
As per the latest amended policy, passport under tatkaal scheme should be applied strictly with the required Annexures. To get signed up these annexures, you don't need to be known personally by the higher required authorities. It is a sort of pre police verification i.e. they provide you the verification certificate prior to apply the passport. For this, they will ask for your all required documents, will check your records and will verify your residing address.
No need to worry, it's their duty. After getting verified they should immediately provide you the verification certificate. Then you can easily apply and get your passport.
@ Soumya Chatterjee:
You should not think too much. Fill the form as per the format printed on application form.
Yes, in PPF (personal particulars form), the surname has to be written first and the format is printed in the same way. So, it's very simple. Just do and follow the format.
That's it.
Hello sir,
I have a doubt with surname in passport application. My name is Jitheesh . My fathers name – Krishnan. My family name _ Pulinkunnath.I have all my certificates with the name – Jitheesh P. While applying for the passport, what do i fill in the surname column ?
Thanks & Regards
Jitheesh
@ Jitheesh.......Hi, to get your passport hassle free, you should provide only that name which is printed on your all documents. The application for passport can be rejected or put on hold due to variation in names. So, its best if you give that name which is available on your all certificates and documents.

@ Sushil kumar....If the passport application is of your father then..you can do the following:
1)Do rectification - Either apply for name correction in Voter id or in Aadhar application. Usually Correction in Aadhar be done fastly.
2) If you don't want to the rectification then, you need to:
a) Provide affidavit for name change (format is available on passport application or can be downloaded from passport authority official website).
b) Get the content of name change published in 2 leading newspaper - 1 english and 1 hindi. They will also need copy of affidavit basis which they get your father's consent to publish the matter.
The affidavit and the 2 leading newspaper's that page where name change is published, will have to be submitted along with passport application. With this, the desired name which is also published, will be published in passport then.
Regards
